All you need to do is write "PSX" after your last name on the PayPal form where it says to write in your address and that's it.  This offer is only good if you order WATCH OUT from us - no other sites will honor this.

Just don't forget to write the PSX after your name or otherwise, the mail room won't know to include the gift dvd.
